  • We have a new exciting opportunity to join our fantastic team as a Transition Development Manager.

This is a full-time, home-based role requiring a full UK driving licence, access to your own vehicle, and occasional travel across England, Wales and Northern Ireland.

This is a national role supporting Mencap teams to establish and deliver high-quality transition services for young people aged 16-18 with a learning disability.

As a Transition Development Manager, you will support operational teams to implement Mencap's Transition Delivery Model, ensuring services are delivered consistently, effectively and in a person-centred way. Through specialist advice, guidance and practical support, you will work collaboratively with colleagues and external partners to develop services that help young people and their families experience positive, well-planned transitions into adulthood.

You will play a key role in embedding best practice, building operational capability and ensuring young people are supported to achieve the best possible outcomes as they move into adult life.

    Key Areas of Accountability:

    • Support operational teams in the mobilisation and delivery of 16-18 transition services in line with organisational standards, policies and agreed models of support.
    • Support services to prepare for inspections, registration requirements and regulatory compliance activity, working alongside Quality and Compliance colleagues where required.
    • Contribute operational expertise to the review and implementation of guidance, procedures, resources and tools that support effective transition services.
    • Work alongside Learning and Development colleagues to identify learning needs and support delivery of training, induction and development opportunities relevant to transition services.
    • Develop effective working relationships with commissioners, local authorities, education providers, health partners and other stakeholders to support successful transition pathways.
    • Support referral pathways, placement planning, mobilisation activities and service reviews associated with the transition model.
    • Promote person-centred practice, positive outcomes and effective partnership working across all areas of service delivery.
    • Gather, analyse and share learning, good practice and service improvement opportunities across transition services.

Please note: Mencap welcomes applications from all suitable candidates and is committed to attracting a diverse workforce. However, all available Certificates of Sponsorship for 2026 have now been allocated, so we are currently unable to offer visa sponsorship. Candidates must already have the right to work in the UK to be considered for this role.

Closing Date: 22nd November. Interviews to commence week beginning 7th December via Microsoft Teams.
