Mobilisations Manager - Personal Support England.
This is a pivotal leadership role at the heart of our growth and impact. As Mobilisations Manager, you will lead the end-to-end mobilisation and transition of people we support with a learning disability into personalised, community-based support and living arrangements enabling individuals to live more independently with tailored support aligned to their needs, preferences and aspirations.
You will ensure every transition is safe, well-planned and person-centred, delivering meaningful outcomes including increased autonomy, choice and improved quality of life. Working in partnership with individuals, families, commissioners, local authorities and health and social care professionals, you will align stakeholders around clear plans, realistic timelines and shared accountability.
This role sits at the intersection of project delivery, person-centred practice, governance and partnership working ensuring mobilisation activity is robust, compliant and outcomes focused. You will provide oversight and assurance that accommodation, staffing models and support structures are in place, that regulatory and quality requirements are met, and that mobilisations transition smoothly into “business as usual” delivery. As growth fluctuates, you will also take a proactive approach to capacity planning: deploying resource to high-impact projects that strengthen mobilisation readiness, embed learning and drive continuous improvement.
You will;
• Lead the end-to-end mobilisation and transition of people from institutional or hospital settings into supported living and community-based services, ensuring safe, well-prepared and person-centred delivery
• Manage day-to-day mobilisation activity across multiple projects, establishing clear governance, defined milestones, realistic timelines and smooth transition into “business as usual” operational delivery
• Own and continuously improve mobilisation frameworks, plans and templates ensuring clarity of roles, responsibilities and decision-making authority, and driving consistency and best practice across the organisation
• Maintain effective risk, issue and dependency management with clear escalation routes; provide regular progress reporting to senior leaders, including Directors and Executive stakeholders
• Lead the co-production of personalised transition plans informed by assessment, referral and support planning processes; embed early engagement and pre-planning within each mobilisation and champion dignity, choice, inclusion and empowerment throughout
• Ensure service readiness prior to commencement including appropriate accommodation solutions, staffing models and support structures maintaining compliance with regulatory standards (including CQC) and working with Quality, Property and Operations teams to ensure services are inspection-ready
• Collaborate with Business Development to support commissioning processes, funding arrangements and resource planning; shape realistic mobilisation plans that support sustainable service models and contribute to the organisation’s future approach to specialist and complex care
• Provide leadership and foster a culture of person-centred practice, accountability and continuous improvement
• Lead and assure the offboarding/decommissioning of services where necessary, ensuring safe transitions, compliance and strong communication with internal stakeholders; support wider transformation and improvement projects during periods of lower growth activity
